[tldoc] tl doc final(?) revision

Dear TeX Live doc workers,

Sorry, but I had to update the TeX Live readme.html and live.tex
documentation once again, for the new (and final, I hope) distribution
plan.  In essence, no more win32 disc.  I replaced the windows
installation section with a brief explanation, but I think the windows
post-install section is still fairly usable, so I left it pretty much
as-is.

I believe Manfred hopes to start making the production discs in a week,
so please do try to get the translations updated in the next few days,
if you intend to update them at all this year.

The only files needed from there are live.tex and tex-live.sty.  
No images are included in the doc any more, by the way.

P.S. Eitan and I have the HTML output mostly working, but are still
ironing out a few last problems before I check those files in.
