When I've had to do this sort of thing I've either done it by hand
using Adobe Acrobat, or worked up a make file which post-processes
the document (write out the page # where the A3 page will go, insert
a blank to keep pagination correct, then read in the # and use a tool
like pdftk to delete the place-holder page and insert the A3 page)
Alternately, if it's an electronic document, rotate it to landscape
and fit it to the page, trusting the people who have access to large
format printers will be able to use Acrobat's ``fit to page'' option
to get a full-size printout.
