Normally these are custom-programmed.

TeX (Plain, LaTeX and probably most other macro packages) could be used 
for this (at my day job we normally used Plain TeX).

There was a bit of a discussion on this recently on the usenet group 
comp.text.tex though --- you could use http://groups.google.com to 
search for it.
