Good guess. Infrastructure updates do *not* remove old files since
*ages*. From tlmgr.pl:

	sub write_w32_updater {
	  my ($restart_tlmgr, $ref_files_to_be_removed, @w32_updated) = @_;
	  my @infra_files_to_be_removed = @$ref_files_to_be_removed;
	  # TODO do something with these files TODO

So now we are somehow hosed ...
