Adjusting both is best. The values' meaning is

   \setuppapersize [<virtual>][<actual>]

were <actual> gives the pdf page dimensions, and <virtual> gives the
usable page area for text, margins, and borders.

The command is for example used for printing in landscape

   \setuppapersize[A4,landscape][A4,landscape]

placing cropmarks

   \setuppapersize[A4][A4,oversized]
   \setuplayout[marking=on]

and for impositioning

   \setuppapersize[A5][A4]
   \setuparranging[2UP]
