No you don't. On windows you *cannot* update tlmgr with it self, because
you cannot overwrite open files.
Normally when you call
tlmgr update --all
on Windows on the Desktop a shortcut is created to a "Final Installation
Step" which has to be run manually. Yes.
Or you fetch
update-tlmgr-latest.exe
from CTAN/system/texlive/tlnet/2008/ and just run it. Then you have all
you need.
It isn't sooo hard. And complain to Microsoft that they cannot write a
decent OS.
