texi2dvi reads etex.fmt, I think, so the question is how this format
was build and if pdftex-dvi.tex was read.

Sure, I did.
teTeX-3.0 shipps with \pdfoutput defined (and set to 0) in etex.fmt and
latex.fmt. I have got a few mails about it, but nobody really complained.
